Patient Safety Incident Response Framework (PSIRF)

Following the publication of the National Patient Safety Strategy in 2019, the Patient Safety Incident Response Framework was published in August 2022 with acute providers expected to implement the framework by September 2023.

PSIRF can only be successful if it is considered a complete transformation programme grounded in human factors and continuous quality improvement. Organisations that merely tweak the serious incident framework will not achieve a reduction in patient harm or improvements in its organisational safety culture.

At Quality Governance Solutions, we can help your organisation by:

  • Supporting diagnostics to inform your Quality Strategy and Patient Safety Incident Response Plan
  • Develop your reporting structure and terms of reference for PSIRF governance.
  • Advise on appropriate team structures to maximise the success of PSIRF.
  • Undertake a review of your PSIRF arrangements to help identify what is going well and identify improvements for consideration.
  • Provide support in the implementation of PSIRF in a way that works for your organisation.
  • Implementation of an in-house human factors hub, with recommendations for market leaders in initial external training.
  • Implementation of mechanisms and tools for sharing learning, restorative practice, improving psychological safety and culture and working with system partners.
  • Supporting Integrated Care Systems with proportionate oversight and shared learning.
  • Promote clinical simulation as an investigation methodology alongside system based tools.
  • Preparing your evidence for future CQC inspections.
